Submitting an inquiry through the Check Eligibility form, sending an email, or calling our clinics does not by itself establish a physician-patient relationship. A relationship is established only when an Evolve physician accepts the referral, conducts a consultation, and agrees to provide care. We respond to all inquiries within one business day; if your situation is clinically urgent, please contact your physician or proceed to the nearest emergency department.

To the extent permitted by Ontario law, Evolve Hyperbarics and its physicians, staff, and affiliates are not liable for any indirect, consequential, or incidental damages arising from your use of this website. Nothing in these terms limits liability for matters that cannot be limited by law, such as gross negligence or willful misconduct.

These terms are governed by the laws of the Province of Ontario and the federal laws of Canada applicable therein. Any dispute arising from or related to your use of this site is subject to the exclusive jurisdiction of the courts of Ontario.
