The fastest way to find out whether HBOT is appropriate for your condition is to complete the form below. An intake coordinator reviews submissions within one business day, and a hyperbaric physician reviews any case that needs clinical input before we respond.
Submissions are PHIPA-compliant and encrypted in transit and at rest. You will receive an auto-confirmation email with a reference number.
For Referring Physicians
Use the Make a Referral page for the dedicated referral workflow including the fillable PDF referral form. Faxed referrals to 1 (888) 906-4268 are triaged within one business day.
