What OHIP Covers
Publicly Funded HBOT in Ontario
The Ontario Health Insurance Plan (OHIP) covers hyperbaric oxygen therapy (HBOT) for 9 specific chronic medical conditions when clinical criteria are met. For these indications, eligible Ontario residents with a valid health card receive treatment at no out-of-pocket cost.
Evolve Hyperbarics is an outpatient hyperbaric network and an OHIP-approved provider. We treat all 8 OHIP-covered conditions across our Ontario clinics, manage all OHIP documentation on your behalf, and confirm eligibility at the consultation visit.
What We Treat

Conditions We Treat at Evolve
- Diabetic lower extremity wounds and other chronic non-healing wounds (per Ontario clinical criteria)
- Failed (compromised) skin grafts and flaps
- Thermal burns (deep partial- or full-thickness, in coordination with a regional burn centre)
- Chronic refractory osteomyelitis
- Avascular necrosis (osteonecrosis), including idiopathic femoral head necrosis
- Radiation tissue damage — soft tissue and bony necrosis, including osteoradionecrosis
- Idiopathic sudden sensorineural hearing loss (within the therapeutic window)
- Crohn’s disease — perianal fistulas, adjunctive to gastroenterology and colorectal care

Eligibility
Eligibility Requirements
To be eligible for OHIP-funded HBOT at Evolve, you must:
- Hold a valid Ontario health card (OHIP)
- Have a confirmed diagnosis matching one of the 9 OHIP-covered conditions we treat
- Meet the disease-specific criteria for that indication (e.g., documented standard care, imaging confirmation, therapeutic window)
- Have no absolute contraindications to HBOT (e.g., untreated pneumothorax, certain chemotherapy agents)
Note: a physician referral is preferred (it speeds the process and helps us coordinate with your existing care team), but it is not required. Our hyperbaric physicians can assess eligibility directly and act as your consulting physician for OHIP purposes. If you reach us without a referral, we see you, then connect you with the relevant doctors or contact your existing care team to coordinate ongoing care.
Getting Started
The Assessment Process

Contact Evolve by phone (1-888-851-4268) or through our Check Eligibility form.

Book a consultation with an Evolve hyperbaric physician. A referral from a physician is preferred but not required.

At the consultation, the Evolve hyperbaric physician confirms OHIP eligibility, reviews your diagnosis and prior management, and screens for contraindications.

If eligible, treatment begins at your preferred Evolve location; if not, we discuss alternatives and, where appropriate, off-label or private-pay options.

We handle all OHIP documentation and coordinate communication with your existing care team.
Some conditions have a biologically defensible rationale for HBOT but are not on the OHIP-covered list. These may be considered on a private-pay basis with explicit informed consent and an honest discussion of the evidence.
This is one of the 9 OHIP-covered conditions Evolve treats. Eligible Ontario residents with a valid health card and a qualifying diagnosis pay $0 out-of-pocket. We prefer a referral from your physician (it speeds the process and helps us coordinate with your care team), but a referral is not required — you can self-refer through our Check Eligibility form. If you reach us without a referring physician, we assess you, then connect you with the relevant doctors or contact your existing care team to coordinate ongoing care.
Acute carbon monoxide poisoning, air or gas embolism, decompression sickness, gas gangrene, severe necrotizing soft tissue infections, and acute crush injury are time-critical hyperbaric emergencies. They are managed at tertiary acute-care hyperbaric facilities (the Toronto General Hospital Hyperbaric Medicine Unit is the regional referral centre), not at outpatient clinics like Evolve.
If you are seeing a patient with a hyperbaric emergency, please contact Toronto General directly. If you are a patient and your situation is urgent, please go to your nearest emergency department or call 911.
HBOT at Evolve is billed to OHIP for Ontario residents only. Patients covered by other provincial plans (e.g., Manitoba Health, MSP) should contact us to discuss reciprocal billing options; in some cases, your home province will reimburse for out-of-province HBOT when the indication is covered and no local service is available.
