Refer a Patient for HBOT
Referring physicians are the heart of how patients get to HBOT. We accept referrals from any Ontario-licensed physician or nurse practitioner — most come from family medicine, vascular surgery, plastic surgery, radiation oncology, otolaryngology, infectious disease, gastroenterology, and the specialty groups managing the OHIP-covered indications. We make it as easy as we can on your end: send what you have through whatever channel suits your workflow, and our intake team will follow up with your staff for anything else we need.

Four Ways to Refer
OCEAN eReferral / EMR
If your clinic uses OCEAN, you can send an eReferral directly to Evolve Hyperbarics through the OCEAN directory. Most major Ontario EMRs (including Accuro, OSCAR, PS Suite, Med Access, and others) integrate with OCEAN — referrals flow straight from your EMR to our intake team without leaving your workflow. We acknowledge OCEAN referrals on the same business day.
If you use a different EMR or eReferral platform, please send what your system supports — we work with your office staff to gather any additional information we need (imaging, prior consult notes, microbiology, current medications) without making you do extra work.
Fax
Send your standard EMR-generated referral form, or our downloadable referral form, to 1 (888) 906-4268. We acknowledge faxed referrals within one business day.
Secure email
Send PHI-protected referrals to info@evolvehbot.ca. We accept OneMail and similar secure-channel referrals from physicians using those services.
Phone (urgent)
Call 1-888-851-4268 for time-sensitive referrals such as sudden sensorineural hearing loss within 14 days of onset, a recently compromised graft or flap, or suspected filler-induced vascular occlusion. We will arrange a same- or next-day consultation whenever possible. The on-call hyperbaric physician is available to discuss the case with you directly.
In Your Referral
What to Include in Your Referral
Send what your EMR includes by default. If anything is missing, our intake coordinator will reach out to your staff — we do not expect you to chase records.
- Patient identifiers and contact information
- OHIP number (where applicable)
- Indication and brief clinical summary
- Relevant diagnostic information (imaging, audiology, vascular studies, microbiology, etc.)
- Prior and current treatment
- Co-morbidities and medications, particularly any that may affect HBOT eligibility (active untreated pneumothorax, certain chemotherapy agents, severe COPD, claustrophobia, pacemaker model if applicable)
- Preferred location (we have six — patient choice typically drives selection)
The Workflow
What Happens After the Referral

Triage by a hyperbaric physician within one business day; urgent referrals same-day.

Our intake coordinator reaches out to your staff for any additional information needed — imaging, prior consult notes, microbiology — so the consultation visit is fully informed.

Consultation booked. Most non-urgent referrals are seen within 1–2 weeks; urgent referrals within 1–3 business days.

In-person consultation with a hyperbaric physician. The patient receives a discussion of the proposed protocol, risks and expected outcomes, and informed consent.

Consultation note sent to you the same week through your preferred channel (OCEAN, fax, secure email).

If accepted for treatment, the patient begins their HBOT course. You receive a mid-course update.

On completion, you receive a discharge summary with outcome measures and recommendations for ongoing care.
A referral from you is preferred — it speeds the process and helps us coordinate around your care plan. But patients can also self-refer through our Check Eligibility form. If a patient we see this way is one of yours, we let you know and bring you into the coordination loop. If they don’t have a treating physician, we assess them and then connect them with the appropriate specialist or family physician after consultation.
