How Funding Works
Four Pathways to Coverage
Hyperbaric oxygen therapy at Evolve Hyperbarics is publicly funded by OHIP for specific chronic medical conditions when ordered by a licensed Ontario physician. For work-related injuries, WSIB may cover treatment. For conditions outside the OHIP-covered list, extended health benefits or private-pay options may apply.

Coverage Pathways at a Glance
OHIP (Ontario Health Insurance Plan)
- Covers the 9 OHIP-approved HBOT conditions Evolve treats — see the OHIP page for the list
- No out-of-pocket cost for eligible patients with a valid OHIP card
- A physician referral is preferred but not required — patients can self-refer through our Check Eligibility form. We coordinate with the patient’s existing care team after assessment, or connect them with the appropriate physician if they don’t have one.
- Eligibility confirmed at consultation by an Evolve hyperbaric physician
WSIB (Workplace Safety and Insurance Board)
- Covers HBOT for accepted work-related injuries that align with covered indications (e.g., chronic non-healing wounds, post-traumatic osteomyelitis, post-reconstructive grafts and flaps)
- Requires an active WSIB claim and authorization from the WSIB nurse consultant or case manager
- Billing handled directly between Evolve and WSIB; no out-of-pocket cost to the worker
Private Insurance / Extended Health Benefits
- Some extended health plans reimburse HBOT for conditions outside OHIP coverage
- Coverage varies significantly by carrier and policy; patients are advised to confirm benefits with their insurer in advance
- Evolve provides itemized receipts with appropriate coding for submission
Private Pay (Off-Label Indications)
- For conditions not covered by OHIP, WSIB, or insurance, treatment may proceed on a private-pay basis
- Used for biologically defensible off-label indications with explicit informed consent acknowledging off-label use
- Pricing is not displayed publicly; please contact us for current rates

How to Find Out What Applies to You

Submit the Check Eligibility form or call our central referral line at 1-888-851-4268.

An Evolve clinician reviews the suspected indication and recommends the most appropriate coverage pathway.

If OHIP-eligible, we coordinate the referral with your physician; if WSIB, we liaise with your case manager; if private, we confirm pricing in writing before treatment begins.

We do not bill patients for OHIP-covered indications.
If your condition is on the OHIP list and you are an Ontario resident with a valid health card, treatment is publicly funded. You will never receive a bill from Evolve for the HBOT sessions themselves.
