Private-Pay / Investigational
Off-Label Indications
Outside the OHIP-covered list, there are conditions for which HBOT has a biologically plausible mechanism and an emerging evidence base. Evolve offers HBOT for selected off-label indications on a private-pay basis — always with a physician consultation, written informed consent, and an honest discussion of the evidence.

Tier 2B

Orthopedic delayed union and non-union, including calcaneal delayed union.
Tier 2A
Inflammatory bowel disease
Ulcerative colitis flares and IBD indications other than the OHIP-covered Crohn’s perianal fistulas.
Tier 2B
Infertility (adjunctive)
Adjunctive HBOT for selected cases, only in coordination with the patient’s reproductive endocrinologist.
Tier 2B
Athletic recovery
For high-performance athletes recovering from injury or competitive exertion, only in coordination with the athlete’s training staff and team physician.
Off-label indications are not on Ontario’s OHIP-covered list and are not recognized indications of the Undersea & Hyperbaric Medical Society. The evidence base ranges from limited to mixed; we are explicit about this on each condition page. Discussion of these conditions is not an endorsement or a recommendation that they be treated with HBOT.
What we don't treat
Being clear about our scope is part of practicing responsibly. Two categories fall outside what Evolve offers — here’s what they are and where to go instead.
Emergency & hospital-based indications
Acute emergencies — carbon monoxide poisoning, decompression sickness, necrotizing infection — need a 24/7 hospital program, not an outpatient clinic. We don’t treat these; here’s the correct pathway.
Off-label conditions we choose not to treat
HBOT is marketed almost everywhere for almost everything. Conditions like autism, MS, and cancer are declined at Evolve on evidence grounds — even for private pay. Here’s where we draw the line, and why.
