What OHIP Covers

When Private Pay Applies

Private-pay HBOT at Evolve is offered for off-label indications — conditions with a biologically defensible rationale for hyperbaric oxygen but which are not on the OHIP-covered list and are not authorized by WSIB. Examples include post-concussive symptoms, long COVID, certain orthopedic non-union scenarios, and select gastrointestinal conditions.

Our Approach to Off-Label Use

Evolve follows a conservative, evidence-anchored approach to off-label HBOT. Before recommending private-pay treatment for any off-label condition, we:

  • Review the patient’s diagnosis, prior treatment, and current symptoms with the consulting hyperbaric physician
  • Discuss the published evidence base honestly — including where evidence is mixed, limited, or absent
  • Confirm there are no contraindications to HBOT
  • Provide a written informed consent document acknowledging off-label use and realistic expected outcomes
  • Quote the total expected cost in writing before any treatment begins

Pricing

Cost and Pricing

We do not display per-session pricing publicly. Pricing depends on:

  • The indication and the recommended number of sessions
  • Whether the treatment is at standard pressure (typically 2.0–2.4 ATA) or modified protocol
  • Whether any additional services (e.g., follow-up imaging review) are included

After consultation, you will receive a written cost estimate covering the entire recommended treatment course. You will not be asked to pay or commit before this estimate is in your hand.

Payment

Payment & Refund Policy

  • Payment is collected per session or as a prepaid block, depending on the treatment course
  • Refunds are available for sessions not yet delivered if treatment is discontinued for clinical reasons or patient choice
  • We do not offer refunds for sessions already delivered, regardless of outcome — consistent with how all medical services operate

What private pay does not include:

  • Diagnostic imaging or laboratory tests outside the scope of the HBOT consultation
  • Specialty consultations from outside physicians
  • Pharmacy or wound-care supplies used outside the clinic visit

Patients sometimes recover part of private-pay costs through their extended health benefits. We provide itemized receipts and a letter of medical necessity to support such claims. However, reimbursement is determined by your insurer; we do not promise or guarantee any particular outcome from a private insurance submission.

Off-Label / Investigational Use Disclosure

The use of Hyperbaric Oxygen Therapy (HBOT) for these conditions is not currently listed by the Undersea and Hyperbaric Medical Society (UHMS) as a standard-of-care indication, and it is not covered by OHIP. It is considered an investigational or off-label use.

Patients pursuing treatment do so on a private-pay basis after a physician-supervised assessment and written informed consent. Evolve Hyperbarics does not represent or imply that HBOT will cure, reverse, or guarantee improvement of these conditions. Individual outcomes vary.

All off-label HBOT at Evolve Hyperbarics is delivered under the supervision of a CPSO-registered physician with hyperbaric training, with reference to published peer-reviewed literature where available.

We will recommend against treatment when evidence does not support it

If your condition does not have a biologically defensible rationale for HBOT, or if available evidence does not support a meaningful expected benefit, we will tell you so plainly. Treatment we do not believe is in your interest is treatment we will not provide.
